Physical& Immunization Requirements

Physical Examinations: 
Physical examinations are required of all pupils completed within one year prior to entrance into Early Childhood, kindergarten or first grade, prior to entrance into 6th grade or upon transfer into Illinois from out of state or out of the country.  This physical exam must be recorded on the State of Illinois Form (Certificate Of Child Health Examination) and returned prior to the start of school.

Vision Examinations: 
For the 2008-2009 school year, a vision examination must be included as part of the health examination required within 1 year prior to entry to kindergarten or upon first entry into an Illinois school. Proof of the eye examination must be completed on the State of Illinois Eye Examination Form and returned to the school health office.

Dental
Dental examinations are required of all children entering kindergarten, 2nd and 6th grades.  Proof of dental examination must be completed on the Dental Examination Form and returned to the school health office.

Lead Screening:  Children aged 6 months through 6 years are required to be assessed and/or screened by their health care provider for lead poisoning.

Tuberculosis Screening:TB skin testing is to be completed as indicted if residing in a designated high-risk area.

Immunizations:  Evidence of immunity against the following diseases must be submitted:

  • Polio: A minimum of 3 doses of polio vaccine is required, the last one given after the 4th birthday.
  • Diphtheria, Pertussis, and Tetanus: A minimum of 4 doses of DPT with the last dose given after the 4th birthday.
  • Measles, Mumps, Rubella: Two doses of live measles vaccine are required; the first dose of measles, mumps and rubella must be given on or after the 1st birthday and the 2nd dose of measles no less than 28 days after the first. Physician documented disease history or adequate measles or mumps antibody titre is acceptable as evidence of immunity. Children who have had rubella disease are not considered immune unless they present evidence of an antibody titre.
  • Hepatitis B:  3 doses of hepatitis B vaccine are required for children entering 5th grade or above and children aged 2 years or older entering a pre-kindergarten program. 
  • Haemophilus Influenza type B: Children aged 2 years or older entering a pre-kindergarten program must submit proof of immunity that complies with the HIB vaccination schedule.
  • Varicella (ChickenPox):  Varicella vaccine is required for any child 2 years of age or older entering a pre-kindergarten program or upon entry into kindergarten through 6th grade for the 2008/09 school year.

Medication Administration at School

Each school has personnel authorized to administer medication to students. However, school employees are not allowed to give prescription or over the counter medication to students without written authorization from the parent and the child's physician. Parents who wish to arrange for medication to be given to their child at school should consult the Parent Handbook and/or contact the school's health office for further details.
